Concerning the Disambiguation Banner:
In case you find use of it for some page and wish to add it to a page, you have but to find it in Insert>Template (as goes for other templates I've made, too) and then fill the given fields.
However, if you wish to fill the Other-Page2 field as well it would be advised to start with " or (the)", as otherwise there will be no blank space or "or" between the first and second other pages.
Also, there is the following alternative, too:

Finally, I also made a Stub Banner, if ever we need one:
